Yes, medical marijuana patients in Arizona can designate a caregiver to assist them.

Patients must first designate a caregiver in their state application. Once the patient's application is approved, the caregiver can then submit their own application through the Arizona Department of Health Services (ADHS) Individual Licensing Portal.

Caregiver Requirements:

  • Minimum age 21 years
  • Must have the patient's registry ID card number, full name, and date of birth
  • Fingerprint card and verification form submitted to the ADHS by mail
  • $200 state caregiver application fee
Here's how to complete the state caregiver application:
  1. Create an account with the ADHS Individual Licensing Portal or log in to your existing account.
  2. Upon login to the Licensing portal, select the Individual Portal tile.
  3. If applying for a new Caregiver License, select the Add a new license tile.
  4. Select New Caregiver Application.
  5. Review if you consent to receive notices from the department, then select I Agree.
  6. In the Caregiver Application search screen, enter your Patient Identification Information and click Search.
  7. Enter Caregiver Identification Information.
  8. Enter Address Information.
  9. Review the 'Consent to do business electronically' agreement.
  10. Place your signature in the box and click Accept to add it to the application.
  11. Click Save & Continue.
  12. Upload Supporting Documents and then click Save & Continue.
  13. Review the application sections before clicking Submit & Go to Payment.
  14. Follow the payment process until the Payment Confirmation page is displayed.
  15. Submit your completed fingerprint package by mail to the ADHS.
