Arizona State Process (Renewals)

Congratulations on your Arizona medical certification renewal. You can follow these instructions to submit a renewal application with the Arizona Department of Health Services (ADHS).

Before you start, have these supporting documents ready:

  • Current photograph of the patient
    • Taken no more than 60 days before the submission of the application
    • Full frontal view of the patient, without a hat or headgear, with a plain white or off-white background
  • Copy of valid ID
  • Completed Medical Marijuana Physician Certification Form (provided by Leafwell)
  • Signed and dated Medical Marijuana Patient Attestation form (required only if you do not consent to an electronic signature)
  • Credit, debit, or prepaid card (Visa or Mastercard only)
  • SNAP documentation (if applicable)

Renewing Your Arizona Medical Marijuana Card

  1. Log in to your account with the Arizona Department of Health Services (ADHS) Individual Licensing Portal.
  2. Select the Individual Portal tile, and then the Marijuana tile.
  3. Select the Patient tile, and then click the Applications tab on the left.
  4. Select the Renewal Application tile.
  5. Review the Renewal Agreement, and then click Agree and Proceed.
  6. Review Patient Information and make any necessary updates. Your information will be automatically populated from your previous application.
  7. Confirm your consent to do business electronically by providing an electronic signature in the box and clicking Accept. If you do not consent to sign electronically, you will need to upload a signed Medical Marijuana Patient Attestation form.
  8. Caregiver Information: select response – If yes, complete available fields with Caregiver information.
  9. Select a response in the Request to Cultivate section, and then click Save & Continue.
  10. Enter Physician’s Information. The Physician Information section must match the signed physician certification form.
  11. Enter Qualifying Health Condition, and then click Save & Continue.
  12. View Other Information Section.
  13. Check the appropriate box for Clinical Studies.
  14. Check the appropriate box for SNAP Eligibility. If yes is selected and SNAP Eligibility documentation is not valid or current, ADHS will send a notification to pay the remaining application amount before the application can be approved.
  15. Select Save & Continue.
  16. Upload all required supporting documentation indicated by the red asterisk (*).
    1. Note: If you provided an electronic signature, you are not required to upload an electronic copy of the Patient attestation.
  17. Upload SNAP documentation if applicable.
  18. Click Save & Continue.
  19. Review the Application sections before continuing to payment.
  20. Once application data is verified, click Submit & Go to Payment to proceed to enter payment information.
  21. Once the payment process is completed, the Payment Confirmation page will be displayed.
  22. Once successfully submitted through the ADHS online system, ADHS will review your application.
  23. If approved before your current card expires, your newly issued card will have an Inactive Renewal status.
  24. Your registry identification card will be electronically available within the Individual Licensing Portal. You may download the card to your mobile device or print it out.
