Congratulations on getting approved for your medical certification. Here's a guide to what you need to do next.
Before you start your application, ensure you have the following items:
I am an adult applying for myself:
- A valid Colorado ID or driver's license.
- If you are applying online, make sure to have a JPEG or PDF copy of your physical, valid Colorado driver’s license or ID. You will upload this to your registration. We do not accept Colorado digital IDs from the MyColorado app.
- Your social security number.
- Caregiver’s registration ID if you are applying with a caregiver.
- Credit card or bank account and routing number to pay the $25 application processing fee.
I am parent or legal representative applying on behalf of a minor
- Primary parent’s Colorado driver’s license or ID. If you are applying online, make sure to have a JPEG or PDF copy of your physical, valid Colorado driver’s license or ID. You will upload this to your registration. They do not accept Colorado digital IDs from the MyColorado app.
- Minor's social security number.
- Primary parent's social security number.
- Certified copy of the minor’s state-issued birth certificate.
- Caregiver’s registration ID if parent is not the caregiver.
- Credit card or bank account and routing number to pay the $25 application processing fee.
- Documentation to prove secondary parent status:
- Secondary parent’s out of state ID if parent lives out of state.
- Copy of secondary parent’s certified death certificate if parent is deceased.
- Copy of a certified court-issued sole custody order if the secondary parent does not have any custody.
- Minor patient consent form
- If you are applying online, make sure to have JPEG or PDF copies of all required documents.You will upload the documents to your registration.
- Patient’s Colorado driver’s license or ID.
- Legal representative’s Colorado driver’s license or ID.

- Patient's social security number.
- Legal representative's social security number.
- Copies of certified legal representative documentation, medical power of attorney documents or certified court orders.
- Caregiver’s registration ID if the legal representative is not the caregiver.
- Credit card or bank account and routing number to pay the $25 application processing fee.

Complete your application online
- Enter your information
- Renewal applicants log into your account and click on your name under "Tracking Inbox: Patient Registration."
- First-time applicants create an account and click “New” under "Tracking Inbox: Patient Registration."
- When you are ready to move to the next step, click "Save & Submit Patient Application."
- Add your provider certification
- Click the “Provider certification” tab
- Click “New”
- Choose your provider certification from the drop-down menu
- Click “Save”
- Optional: Choose your cultivation information
- Click the “Cultivation information” tab
- Click “New”
- Select your cultivation option and complete the required fields
- Click “Save”
- If you are applying with a caregiver, make sure that you have your caregiver's registration ID number. Your caregiver will need to accept a caregiver agreement in order to be listed on your card.
- Pay
- The application processing is fee is $25. It is non-refundable and is required each time you submit an application.
- To pay with a credit card, debit card, or electronic check
- Click the "Payment" tab
- Click "New"
- Choose "Click here to pay." This will open a new window where you will enter your payment information.
- To pay with a fee waiver
- Submit a copy of your certified Colorado tax return for the current tax year. You can get this from the Colorado Department of Revenue.
- You may qualify for a fee waiver if your household income is 185% of the Federal Poverty Level or less. Review annual adjusted household incomes here: https://aspe.hhs.gov/poverty-guidelines
- Your application is reviewed
- Applications submitted online with all correct information are approved in 1-3 business days.
- Applications take longer to process if they are submitted with incorrect information or expired documents.
- Once your application has been processed you will get an email to the address you used to register.
- Print or display your card
- Once approved, you can access your card through your online account.
- Click on your name under "Tracking Inbox: Patient Registration."
- Click the “print card” button to print your card or display it from a mobile device after you have been approved.
- If you applied online you will not receive a card in the mail.
- Once approved, you can access your card through your online account.
NOTE:
- After you have been approved by your Leafwell physician you have 6 months from the date your provider certification was issued to complete the rest of your application.
- The issue and expiration dates on your card will be based on the day your application was approved.
