Congratulations on getting approved for your medical certification.
Per state regulations, you must submit your state application within 90 days of certification.
Here's a guide to what you need to do next.

- Register with the state
- Click here to begin.
- Click on New Program Participants.
- Enter your name and email and create a password.
- After completing this one-time registration process and verifying your account via e-mail, you can log in.
- Log In
- Log in using the e-mail address and password you established during the registration process.
- Select Patient Application under the Applications tab.
- Upload a photo of yourself.
- Fill in all the necessary boxes including driver's license/state ID information.
- Fill in the Patient Release Request Form
- Fill in your name, qualifying condition, and Physician's information:

- Date and electronically or manually sign.
- Fill in your name, qualifying condition, and Physician's information:
- OPTIONAL: Fill out Caregiver Authorization form if applicable
- The caregiver is required to do a State and Federal background check which is good for three years.
- The caregiver must fill out a separate application.
- Fees
- Patient and caregiver application fees are $50 for one year, $75 for two years, or $100 for three years.
- Patient’s Attestation Statement
- Read the statement and select the radio button to confirm each of the additional statements.
- Click ‘+Upload’ to upload a copy of your identification.
- Once all sections are complete, review the application, sign/date the application, and click ‘Save’ to save and transmit the application.
Please note: You have 90 days from the date of your certification to complete the state application. Failure to do so may result in additional fees.
