Vermont State Process (Renewals)

Congratulations on your recent renewal! If you're already a Vermont patient and do NOT yet have an online portal account, you will need to email the program (ccb.med@vermont.gov) and request that they set one up for you.

Renewing Your Vermont Medical Marijuana Card

Log into your existing account with the Vermont Cannabis Control Board.
Download your Health Care Professional Verification Form - this will be emailed to you after your appointment with Leafwell. Please check that all the information is correct before submitting it to the state.
Complete all parts of the application form.
Pay the $50 application fee.
Upload a photo that will be used for your card.
If required, complete the caregiver application too - this can be found on the same page as the patient form.
The state will process complete applications within 30 days and mail you your card.
